The incumbent is responsible for developing a medical-social collaboration model/ system under District Health Centre (DHC) scheme, planning, developing and compiling the social service standards and guidelines for DHCs, researching and exploring local social service resources and developing the comprehensive local social service resource database, assisting in developing a social service resource system at the community level for enhancing accessibility to the resources by DHC clients in need, and monitoring the performance of the operators of the DHC Scheme.

Candidates should –

• (a)
  • (i)have registered as a registered social worker (category 1) under the Social Workers Registration Ordinance, Chapter 505; or
  • (ii)have a Certificate of Registration (Part I / II) issued by the Nursing Council of Hong Kong and a valid practising certificate issued by the Nursing Council of Hong Kong; or
  • (iii)have registered in Hong Kong as a Physiotherapist in Part Ia of the Register of Physiotherapists under the Supplementary Medical Professions Ordinance (Cap. 359) and a valid Practising Certificate issued by the Physiotherapists Board of Hong Kong; or
  • (iv)have registered in Hong Kong in Part I of the Register of Occupational Therapists under the Supplementary Medical Professions Ordinance (Cap. 359) and a valid Practising Certificate issued by the Occupational Therapists Board of Hong Kong; or
  • (v)be an active member of the Hong Kong Institute of Speech Therapists under the Accredited Registers Scheme for Healthcare Professions of Department of Health; or
  • (vi)be an active member of the Hong Kong Academy of Accredited Dietitians under the Accredited Registers Scheme for Healthcare Professions of Department of Health;
• (b)possess at least 10 years’ full time post qualification work experience in public health related services or social services, preferably in the field of medical social collaboration, of which 5 years should be full time managerial and supervisory experience

• (d)possess leadership skills including negotiation, problem solving and relationship building with service providers, community groups and other related parties; and
• (e)have met the language proficiency requirements of Level 2 or above in Chinese Language and English Language in the Hong Kong Diploma of Secondary Education Examination or the Hong Kong Certificate of Education Examination (HKCEE)
